Service Contract
Please Read before signing


Two keys are required for access.

Payment Policy:

New Clients:
- 100% payment is due at the reservation.

Existing Clients:
- 50% payment is due at the reservation.
- 50% payment is due at the first date of service.

If a late payment occurs, the new client policy will apply. A 5.00 late fee per day will be added to unpaid balances after 7 days of return. Unpaid balances exceeding 15 days will be sent to collections, and the pet sitter/client relationship will be terminated.

A $35.00 processing fee will be charged for insufficient funds.

Payment can be made in the following forms:
- Cash
- Check
- Zelle
- Credit card

Cancellation Fees:

Early Return:
- No refund is given.

Overnights:
- A 50% credit is given if canceled within 14 days.
- Please note that overnight stays are from 6:00 PM to 4:00 AM. If you require visits during the day, a separate charge will be incurred per visit.

Holidays:
- No refund or credit is given if canceled within 14 days.
- A 50% credit is given if canceled within 30 days.

Client agrees to be financially responsible for any additional visits required, special trips, pet food, supplies, veterinary visits, and any injury inflicted on the pet sitter by your pets while in our care or training. If contact cannot be established with the client within 24 hours of return via phone, email, or text message confirming the return, an additional emergency visit will be made, and clients will be responsible for additional charges.

Alicia’s Pet & Home Care, LLC is not liable for damage to your home or property caused by your animals during your absence. We are also not responsible for the injury, disappearance, death, or fines of your pets with access to the outdoors. We are happy to water plants or tend to your yard as per your instructions, but we are not responsible for the well-being of your plants.

The client authorizes the pet sitter to permit their employee or contractor to enter the client’s home in the event of a personal emergency or illness to the pet sitter. The client has arranged for a qualified substitute to fulfill the service contract.

If any individual other than the contracted pet sitter enters the client’s home or property, all responsibility and liability on the part of Alicia’s Pet & Home Care, LLC pet sitting is null and void. The client agrees not to allow any other person to feed, medicate, groom, or care for pets without the consent of Alicia’s Pet & Home Care, LLC.

The client fully comprehends the terms of this contract and, by signing it below, assumes full responsibility for prompt payment of fees upon reservation of services. The client authorizes this signed contract as approval for future services and acknowledges that fees and policies may be subject to change at any time without prior notice.

Vet Release
Alicia's Pet & Home Care, LLC
Emergency Veterinary Release Agreement

In the event that any of my pets or large animals appears to be ill, injured, or at significant risk of experiencing a medical problem at the start of service or while in the care of Alicia's Pet & Home Care, LLC I give permission to Alicia's Pet & Home Care, LLC to seek veterinary service from a veterinarian or a veterinary clinic. Other veterinarians or emergency care clinics chosen by the pet sitter are acceptable.

I ask Alicia's Pet & Home Care, LLC to inform the attending clinic or veterinarian of my requested total diagnosis and treatment limit of $ __________ per pet/all pets (most common values are $200, $1000, or unlimited). I understand that efforts will be made to contact me regarding any treatments, illness, injury, or potential problems as soon as the condition is deemed not life threatening and/or contact is possible. I understand that Alicia's Pet & Home Care, LLC providers work hard to prevent accidents and injuries, and that such problems may occur no matter how well a pet is cared for. I agree to allow Alicia's Pet & Home Care, LLC providers to use their best judgment in handling these situations, and I understand Alicia's Pet & Home Care, LLC and its staff assume no responsibility for the actions and decisions of the veterinary staff, the health, or death of my pet(s).

I will assume full responsibility for the payment and/or reimbursement for any and all veterinary services rendered, including but not limited to diagnosis, treatment, grooming, medical supplies, and boarding. Such payments will be made within 10 days of each incident.

I further authorize Alicia's Pet & Home Care, LLC and my primary veterinarian(s) to share all of the medical records of all of my animals with veterinary clinics in an emergency in the interest of providing the best care for my ill or injured animal (s).
Every dog, cat, and horse at the site of service will be current (per my veterinarian's recommendations) on its rabies vaccinations prior to the arrival of any caregiver. I will also make arrangements to guarantee that each animal will remain current on its rabies vaccinations throughout each service visit period.

I agree to notify Alicia's Pet & Home Care, LLC of any signs of injury or possible illness before any visit as soon as the condition appears. Alicia's Pet & Home Care, LLC reserves the right to cancel service at any location where a pet with a potentially infectious condition exists. Alicia's Pet & Home Care, LLC strives to provide clean, safe service to each of our clients. In doing so. Alicia's Pet & Home Care, LLC strongly recommends that each pet and large animal vaccinated, dewormed, and protected from harmful insects according to veterinarian recommended standards.

This agreement is valid from the date below and grants permission for future veterinary care without the need for additional authorization each time Alicia's Pet & Home Care, LLC cares for one or more of my pets. I understand that this agreement applies to all of the pets and large animals within Alicia's Pet & Home Care, LLC. In signing this contract, I agree that I have sole authority to make health, medical and financial decisions regarding the animals that will be scheduled to receive service.
